Project Description
The SNP Welfare Foundation Medical Center is a community-driven healthcare initiative dedicated to enhancing the well-being of underserved populations. Established with a vision to make quality healthcare accessible to all, the center offers a range of essential medical services to low-income patients, including medical checkups, pharmacy services, and physiotherapy and rehabilitation services, all available at either no cost or at a reduced fee. Additionally, the center conducts health awareness programs to educate the community on preventive healthcare practices. With a dedicated team of healthcare professionals and a patient-first approach, the center serves as a beacon of hope for countless individuals in the region.
Project Objectives
- Medical Checkups: Ensure early detection and management of health issues through free medical checkups by qualified physicians.
- Pharmacy Services: Provide free and discounted medications and comprehensive vital signs checkups to support holistic patient care.
- Physiotherapy and Rehabilitation Services: Offer free and discounted physiotherapy and rehabilitation to aid recovery from injuries and manage chronic conditions.
- Health Awareness Program: Raise awareness about preventive healthcare, healthy living, and disease management through educational sessions and materials.
Project Activities
- Medical Checkups: A qualified physician is available at the medical center on a fortnightly basis. Responsibilities of the physician at the center include conducting thorough initial health assessments, reviewing medical history and current health concerns, measuring and recording vital signs, performing detailed physical examinations, and orderand interpreting diagnostic tests. Availability of the physician at the medical center has ensured ongoing care for patients with chronic conditions through regular follow-up appointments. When necessary, the physician referspatients to specialists to ensure comprehensive care. Availability of the physician has also enabled the medical center to educate patients on health conditions, treatment options, and preventive measures.
- Pharmacy Services:A qualified pharmacist is available at the medical center on a daily basis. Responsibilities of the pharmacist include dispensing medications to the patients, ensuring they have access to necessary treatments. The pharmacy at the medical center offers free and discounted medications to eligible low-income patients. The pharmacist also conducts comprehensive vital signs checkups, including measuring blood pressure, heart rate, blood glucose levels, temperature, and respiratory rate, and BMI, to monitor patients’ overall health. This holistic approach supports the management of treatment plans and helps in early detection of potential health issues. Additionally, the pharmacist educates patients on medication usage, potential side effects, and preventive health measures. Regular follow-up appointments are scheduled to ensure ongoing care and to adjust treatment plans as needed.
- Physiotherapy and Rehabilitation Services: The medical center is equipped with necessary facilities to support basic levelphysiotherapy and rehabilitation services. A qualified chiropractor and a physiotherapist are available at the center on an appointment basis. They provide one-on-one physiotherapy assessments and treatments, including joint mobilization, stretching exercises, and posture correction techniques. Additionally, they offer personalized advice on exercises, posture improvements, and lifestyle changes to support long-term health. Rehabilitation services include tailored programs to help patients recover from injuries, surgeries, and manage chronic conditions. These programs focus on improving mobility, strength, and overall functional abilities. The team also conducts regular follow-up sessions to monitor progress and adjust treatment plans as needed, ensuring optimal recovery and ongoing support for patients.
- Health Awareness Program: Medical center organizes sessions with healthcare professionals to discuss the importance of preventive care, nutrition, physical activity (including exercise), and lifestyle changes for better health. The medical centerprovides handouts and pamphlets with tips on preventive care, nutrition, lifestyle modifications, and physical activity. At SNP Welfare Foundation Medical Center, we also provide educational material on physiotherapy and rehabilitation, includinginjury prevention, posture correction, and physical activity.
